Staff – CI Graphic Request Form

For internal use only – CI employees can request a graphic design from the media department. This form will help the project to run smoothly and efficiently!

The survey will take approximately 5 minutes to complete. The media team is delighted to serve your department’s request. Please use this form to detail the kind of graphic need you have.

Please also allow for a minimum of four business days to process any requests. What this means is the project will be under review if not completed by that time.

It depends on the size of the project and other factors, such as other requests that have been submitted prior, company-wide events, and a host of other projects that are in development unrelated to the media team’s graphic service. Please understand that it is our priority to do these requests as fast as our schedules and other responsibilities allow.

Because it is our intention to serve you to the best of our ability, please submit this completed form for us to be able to meet the needs of your department in a timely manner.
